A new NCLEX-PN test plan took effect April 1, 2026, and the most useful thing about it is what did not change: the percentage ranges for every Client Needs category and subcategory are identical to the 2023 plan. No weights moved, no categories were added, and study material built against the previous blueprint is still aimed at the right targets. What the plan does give you — and what most PN candidates never actually read — is a precise map of where your items come from, and one category is far larger than the rest.

Clinical judgment is measured explicitly
Beyond the Client Needs categories, the plan measures clinical judgment directly: 18 case-study items — three sets of six — plus roughly 10% stand-alone judgment items, selected depending on exam length. Each case-study set shares one unfolding client presentation and walks the same six-step chain: recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, evaluate outcomes. Those items look different from anything on a school exam, which is why format practice matters as much as content review — the thinking is familiar, the packaging is not.
